HOW TO APPLY
INDEPENDENT LIVING
To assist you with the application process, please download the Application Checklist.
For an application to be deemed complete, it must include a copy of the most recent CRA Notice of Assessment, and two forms completed by your physician: MOST (Medical Order for Scope of Treatment) Form, and Physician’s review.
Once completed, either drop off or mail the completed application and documents to Seton Villa. Alternatively, you may email the complete application to Laura at lperkins@setonvilla.com.

ASSISTED LIVING
Seton Villa has 28 Assisted Living suites designated for seniors with low to moderate incomes who need assistance with daily living. Funded by Fraser Health and the provincial government’s Independent Living BC (ILBC) program, eligibility is determined by a Fraser Health Case Manager assigned to Seton Villa.
